LDC Participant Liability Waiver

I agree to show proof of vaccination no later than 14 days before arrival at the event or other documentation for exemption. I agree to abide by all health and safety guidelines and recommendations promulgated by federal, state, and local government agencies and officials, the event venue, and ESA. This includes, but is not limited to, respecting restrictions set in place during the event that may consist of wearing a mask at all times in indoor public areas, engaging in appropriate physical distancing, and abiding by instructions on certain activities that carry higher COVID-19-related risk. I further agree that I will not attend the Life Discovery Conference if I become ill, being exposed to the COVID-19 virus, or exhibit any symptoms of the COVID-19 virus before the event.

By signing this waiver, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 while at the Life Discovery – Doing Science conference at Tallahassee, FL, or associated field trip sites and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I forever waive my right to hold ESA, its Directors and staff, event organizers, event partners (Botanical Society of America and Society for the Study of Evolution), the event site (Florida A&M University), and field trip hosts and sites liable for any loss or damages arising from such exposure or infection.
